The Simmonds test (also called the Thompson or Simmonds-Thompson test) is used in the examination of the lower extremities to test the rupture of the Achilles tendon. The patient is lying face down with his feet hanging over the edge of the bed. If the test is positive, there is no leg movement (normally plantarflexion) when pressing the right calf, indicating a possible outbreak of the Achilles tendon.

Interpretation of results

Simmonds' test has a positive predictive value of 13.7 and a negative predictive value of less than 0.1.

Recent research has shown that while the Simmonds test is an accurate detector of the outbreak of achilles, it can not distinguish between partial tears (gastrocnemius tears or the only part only) and complete tears from both parts.

History

It was named after Franklin Adin Simmonds (1911-1983), a British orthopedic surgeon at Rowley Bristow Hospital, Surrey.
